Registration

DMV Certificate of Attendance

Students under the age of 18 who wish to apply for a learner's permit or driver's license must submit to the DMV office at the time of application a completed DMV-301 form signed by a school official. This form will verify that the applicant has attended at least 90 percent of the school days in the current school year -- or they do not have more than 10 unexcused absences.

DMV-301 forms can be submitted to the Registrar, Susie Cortez, no more than two weeks prior to the student’s scheduled DMV appointment. Please complete the Student Information fields prior to submitting the form. Allow 48 hours for the registrar to complete the School Use section.

Requesting Transcripts

Current students can access unofficial transcripts through Infinite Campus. Current students who need official transcripts can request them through our registrar, Susie Cortez, cortens@nv.ccsd.net, or through Parchment, transcript.ccsd.net.

Parchment is a secure digital platform that allows students, alumni, and third-parties (e.g. colleges, employers, etc.) to request transcripts and student records. Click here for assistance with Parchment requests.

K-12 schools requesting student records should go to k12records.ccsd.net.
